Yesteday for the first time I saw a healer negociating a payment with a fallen to raise him. And it was for a good reason since this top fighter has a reputation for being avaricious.

If it was Andrea, that's her "thing". She does that all the time, sort of like Malkor. How can you tell if a fighter is "avaricious?"

I think demanding payment for healing works only in an RP sense, if you have any interest in actually being invited on hunts. On the other hand, getting tips is the best.
(My girls have taken to calling Artaks "pretty bags of mostly money" (from a TNG inorganic alien that called people ugly bags of mostly water).)

Sometime I get a huge tip (as high as 100c) even for a mondane heal.
Is there some form of etiquette outside a hunt, the basic service (à la Stora) or a weak exile?

In the early days, the custom was that healers were tipped whenever they healed, be it a top off in town center, a rescue, or maybe at the end of the hunt for their services. Tips needn't be large; topping off could be worth anywhere from 1 coin onward.
Tips were also given to chainers, who were once a rare breed, for their chain maintenance. This still happens but on a very small scale.
At this point, tips are not so deeply ingrained in the culture, but I try to tip (healers and chainers) when I have a few spare coins and am feeling appreciative.
It is still expected that Mystics be tipped for illusion-opening, which is strange, since it seems like one of the least-involved services they can render. Surely a difficult teleportation feat, locate, or just even really good boost is as valuable if not more. But, I suppose the payment in this case is for the Mystic sitting around and waiting on whichever party is wanting the door opened.

My fighter tries to tip when she can (which isn't often, yet) for healing that happens as not part of a hunt. So if I have to run back to town, or get healed by a passerby somewhere, or especially if someone has taken the time to scrape my poor carcass off the ground somewhere. If the healer is hunting with me they're getting part of the coins that are generated, so I don't tip them.

FrozenHam almost never gets tips (maybe once every other ooc-week), and he does a lot of in-town healing and topping-off. He doesn't complain, though. When your only friend is a pretzel, there's not much lower you can go.

My tipping guidelines:
- TC Heal 10c - 25c depending on my coins in pocket and how many people are healing me
- Seeing a Mystic Online 25c - 50c
- Mystic on my Hunt 25c - 50c
- Mystic does something special 50c - 100c
- Healer/Fighter Chain rescue 50c -100c
- Healer comes to my rescue 50c - 250c depending no difficulty
- Healer helps me on boring hunt where they get little or no exp/coins 50c - 250c
- Mystic does something great (makes EPS, Amulet, Teleport) 100c - 250c
For any fighter that has a good income they should be tipping. I have also tipped up to 1000c for extraordinary things. I generally don't tip healers on a hunt where they get exp or coins. The good reason to tip is we lack good healers sometimes and it is good to have Mystics around. Last night CoNGA could not find a healer for our hunt. Not ONE online or via sinstone. Sad....
